We strive to help you understand the benefits your insurance plan offers. Navigating insurance can be complex, even for us, and while we do our best to make the process smooth for you, there are important things to keep in mind:

  • Verify Your Benefits: You are responsible for knowing the details of your insurance plan. We recommend using our template to track your benefits. As a courtesy, we will attempt to verify your benefits before treatment and at the start of each new plan year. Keep in mind, however, that plans may change, so it's important to recheck your coverage at renewal. Any information we provide regarding your policy is a quote of benefits and not a guarantee of payment.

  • Copay and Coinsurance: Payment is due at the time of service. Insurance reimbursement rates can vary, so you may receive a refund, a credit, or a bill for any remaining balance.

  • Financial Responsibility: You are responsible for payment of services received. If there is a difference between what is billed and what your insurance covers, we will notify you. By using your insurance, you agree to be responsible for any outstanding balance if your insurance does not reimburse us.

  • Plan-Specific Requirements: Insurance coverage details vary. For MVA and Workers' Compensation claims, a doctor's referral is generally required. Personal health insurance plans may require a prescription or referral for massage therapy. Some plans may also need pre-authorization before treatment is approved.

  • Out-of-Network Benefits: Although it’s rare, some insurance plans may cover out-of-network massage therapy. If you're interested in using out-of-network benefits, please contact us for more information.

  • Important Reminder: Always contact your insurance company directly to verify your outpatient physical therapy benefits. Use the customer service number on the back of your insurance card(s). While we also verify benefits as a courtesy, the information received is not a guarantee of coverage. You are responsible for understanding your insurance contract, and if any discrepancies arise—such as exhausted benefits or policy changes—you are liable for any charges not covered by insurance. Deductibles and out-of-pocket maximums only reflect claims processed by your insurance company.

  • Payment Policy: We submit claims daily, and payment is required at the time of service.
